  • Everyone seeking access to good health, disease self-management and prevention education programs: has access, readily available in local communities: where they live, work, play, worship and retire.

  • Self-Care Education available without regard for ability to pay for services

  • Access begins with Health Insurance, and Means Access to Wellness, Preventative Care, Screening. Early Detection, Outreach and Support Services, Navigation of Health Systems and Engagement with Care Teams.
