Dear Colleagues,
Welcome to our National Training & Technical Assistance Center (NTTAC).
I have worked in the Health Care Industry (Population Care
Management for Chronic Diseases) for over three decades.
During that time, I "figured out" How To: improve clinical health outcomes for chronic diseases in underserved populations.
Community Health Centers {CHCs) are best suited to provide care and meet the needs of underserved populations. We support CHCs by increasing access to trained Community Health Workers, to provide community outreach and prevent provider burnout (from not being able to meet the needs of their underserved population). This support is combined with Training and Technical Assistance (TTA) to providers, front line staff and administrators: to strengthen CHCs capacity for improving Clinical Quality and Patient Safety. Our Goal is for CHCs to perform at their highest level as evidenced by HRSA's award of badges representing Clinical Quality and Patient Safety.
Welcome to IPDC - NTTAC. We utilize a Collective Impact Framework Approach, a Network of Partners and Resources made available to CHCs at no cost to achieve Health Equity, Improve Clinical Quality of Care and Patient Safety. We look forward to scaling our support for CHCs to achieve performance improvement badges as evidence of Clinical Quality and Patient Safety.
